Fees for registration include Home Materials.

Registration: Complete registration information below & mail with registration fee to address below or email with complete credit card information. Return this form and registration fee (non-refundable) by appropriate deadline. Late Registrants may be required to pay an additional fee to cover extra shipping expenses. Registrations received without a registration fee will not be processed.

Class maximum is 12 children per class, 10 per class in certain locations.

Family Discount: 10% off family total for more than one child enrolled per family. This does not apply to "Family Time" as it is an already reduced tuition. Sibling Materials are available for families with 2 children enrolled in the same class.  Please specify requests for sibling materials when registering.

Classes held at 96 Plaza Drive (next to Blockbuster Video) in Lawrenceburg & the Danville Community Arts Center (Main St) in Danville.

Authorization/Agreement
I am registering my child for the Kindermusik program with Lawrenceburg Academy of Performing Arts. I agree to pay scheduled tuition and understand that I am responsible for the entire cost of tuition as published on the class calendar page or pay the appropriate late fees. Registration Fee is due with form for space to be held for your child. I agree to pay a $25 fee in the event that my bank, due to non-sufficient funds, does not honor my check. I/or my spouse agree to accompany my child to each class.

Payment Options:
Tuition payments are to be made in full by the 1st class. Cash, check and credit card are accepted. Refunds on tuitions will not be given after the 3rd class. Refunds are made at 60% of remaining balance and will be given in KinderBucks. KinderBucks must be used by 1 year of the date issued. KinderBucks are non-transferable.
